Physical Examination & Immunizations: Illinois State Law requires a complete physical examination for a student before he/she enters school. Completed forms for Pre-K, Kindergarten, 6th grade, and all new students coming to Holy Trinity, must be turned in before the first day of school. Failure to meet this requirement will result in the student not being admitted to school. Immunizations for all students must be up to date.

Eye Examination: Incoming Kindergarten students, as well as all new students to Holy Trinity must have an eye examination completed and forms filled out before he/she enters school.

Dental Examination: Students entering Kindergarten, 2nd and 6th grades are required to have an oral health examination performed by a licensed dentist within the previous nine months according to state law. Proof of this examination, documented on the required State form, must be presented to school by the first day of classes.
